摘要:
目的 比较药涂球囊与金属裸支架治疗下肢动脉硬化闭塞症的疗效。方法 选择郑州大学附属洛阳中心医院2016年1月至2017年12月收治的63例下肢动脉硬化闭塞症患者为研究对象,根据治疗方法将患者分为药涂球囊(DCB)组(n=31)和金属裸支架(BMS)组(n=32),DCB组患者给予紫杉醇药涂球囊扩张成形术,BMS组患者给予BMS植入术。比较2组患者手术即刻和术后1 a血管通畅率、踝股指数(ABI)及术后1 a靶病变血流重建率、主要肢体截肢率。结果 DCB组患者术后即刻和术后1 a血管通畅率及术后1 a靶病变血流重建率分别为100.00%(31/31)、93.55%(29/31)、3.23%(1/31),术后即刻和术后1 a ABI分别为0.32±0.24、0.72±0.29;BSM组患者术后即刻和术后1 a血管通畅率及术后1 a靶病变血流重建率分别为100.00%(32/32)、62.50%(20/32)、43.75%(14/32),术后即刻和术后1 a ABI分别为0.39±0.28、0.56±0.35。2组患者术后1 a均未出现死亡或主要肢体截肢,DBC组患者未出现明显的药物不良反应。2组患者术后即刻血管通畅率比较差异无统计学意义(χ2=1.785,P>0.05);术后1 a,DCB组患者血管通畅率高于BMS组,靶病变血流重建率低于BMS组(χ2=8.780、17.443,P<0.05);2组患者术后即刻ABI比较差异无统计学意义(t=-5.684,P>0.05);术后1 a,DCB组患者ABI高于BMS组(t=6.505,P<0.05)。结论 DCB在术后1 a血管通畅率、靶病变血流重建率及ABI方面较BMS有明显的优势,可以作为治疗下肢动脉硬化闭塞症的重要手段。
Abstract:
Objective To compare the efficacy of drug-coated balloon and bare metal stent in the treatment of lower extremity arteriosclerosis obliterans.Methods Sixty-three patients with lower extremity arteriosclerosis obliterans in Luoyang Central Hospital of Zhengzhou University from January 2016 to December 2017 were selected as subjects.The patients were divided into the drug-coated balloon (DCB) group (n=31) and the bare metal stent (BMS) group (n=32) according to the treatment method.The patients in DCB group were treated with DCB dilatation,the patients in BMS group were treated with BMS implantation.The immediate vascular patency rate after operation,the vascular patency rate at one year after operation,the blood flow reconstruction rate of target lesions at one year after operation,ABI at immediateness and one year after operation and the major limb amputation rate of patients were compared between the two groups.Results In the DCB group,the vascular patency rate at immediateness and one year after operation,the blood flow reconstruction rate of target lesions at one year after operation was 100.00% (31/31),93.55% (29/31) and 3.23% (1/31) respectively;the ABI at immediateness and one year after operation was 0.32±0.24 and 0.72±0.29,respectively.In the BSM group,the vascular patency rate at immediateness and one year after operation,the blood flow reconstruction rate of target lesions at one year after operation was 100.00% (32/32),62.50% (20/32) and 43.75% (14/32)respectively;the ABI at immediateness and one year after operation was 0.39±0.28 and 0.56±0.35,respectively.No death or major amputation occurred in both groups within one year after operation,and no obvious adverse drug reactions occurred in DBC group.There was no significant difference in the immediate vascular patency rate of patients between the two groups after operation(χ2=1.785,P>0.05).The vascular patency rate of patients in the DCB group was significantly higher than that in the BMS group after operation,and the blood flow reconstruction rate of target lesions was significantly lower than that in the BMS group at one year after operation (χ2=8.78,17.44;P<0.05).There was no significant difference in the immediate ABI after operation between the two groups (t=-5.684,P>0.05);The ABI of patients in the DCB group was significantly higher than that in the BMS group at one year after operation (t=6.505,P<0.05).Conclusion Compared with BMS,DCB has obvious advantages in the vascular patency rate,the blood flow reconstruction rate of target lesions and ABI after one year of surgery;DCB can be used as an important treatment for lower extremity arterial occlusive diseases.
