摘要:

目的　观察河南省平顶山矿区过敏性皮肤病患者的皮肤斑贴试验结果，分析致敏原，为过敏性皮肤病的诊断和治疗提供依据。方法　选择2016年8月至2018年1月平煤神马医疗集团总医院收治的过敏性皮肤病患者330例，采用皮肤斑贴诊断试剂盒进行斑贴试验，分析过敏原分布情况。结果　330例过敏性皮肤病患者应用45种标准系列抗原斑贴进行试验14 850贴次，阳性反应953次，其中，斑贴试验阳性率排名前5位的变应原分别为：氯化镍（45.15%）、十二醇硫酸钠（38.79%）、氯化钴（25.76%）、硫酸镍（20.00%）和重铬酸钾（15.45%）。330例过敏性皮肤病患者中，皮肤斑贴试验阴性74例，阳性256例，阳性率为77.58%（256/330）。256例皮肤斑贴试验阳性患者中，女性159例（77.94%），男性97例（76.98%），男性与女性过敏性皮肤病患者皮肤斑贴试验阳性率比较差异无统计学意义（χ²=0.281，P>0.05）。结论　河南平顶山矿区过敏性皮肤病的致敏原主要为氯化镍、十二醇硫酸钠、氯化钴、硫酸镍和重铬酸钾等，分析该地区过敏性皮肤病的主要致敏原对该疾病的诊断和治疗具有一定的指导意义。

Abstract:

Objective　To observe the skin patch test results of patients with allergic dermatosis in Pingdingshan mining area of Henan province，and analyze the allergens，so as to provide the basis for the diagnosis and treatment of allergic dermatosis.Methods　A total of 330 patients with allergic dermatosis in the General Hospital of Pingmei Shenma Medical Group from August 2016 to January 2018 were selected as subjects.The patch test was carried out with skin patch diagnostic kit to analyze the distribution of allergens.Results　A total of 330 patients with allergic dermatosis received 14 850 skin patch tests with 45 standard series antigen patches，and 953 times were the positive reactions.The top five allergens in skin patch test positive rate were nickel chloride （45.15%），sodium dodecyl sulfate （38.79%），cobalt chloride （25.76%），nickel sulfate （20.00%） and potassium dichromate （15.45%） in turn.Among 330 patients with allergic dermatosis，74 cases were negative and 256 cases were positive in skin patch test，the positive rate was 77.58% （256/330）.There were 159 women （77.94%） and 97 men （76.98%） in the 256 patients with positive skin patch test.There was no significant difference in the positive rate of skin patch test between male and female patients with allergic dermatosis （χ²=0.281，P>0.05）.Conclusion　The main allergens of allergic dermatosis in Pingdingshan mining area of Henan province are nickel chloride，sodium dodecanol sulfate，cobalt chloride，nickel sulfate and potassium dichromate.The analysis of the main allergens of allergic dermatosis in this area has certain guiding significance for the diagnosis and treatment of this disease.
