摘要:
目的 探讨血浆溶血磷脂酸(LPA)及脂蛋白相关磷脂酶A2(LP-PLA2)水平在评价冠状动脉粥样硬化性心脏病(CHD)患者冠状动脉病变程度中的临床意义。方法 选择2015年7月至2017年7月驻马店市中心医院收治的238例CHD患者为研究对象。根据临床症状及检查结果分为CHD组(n=186)和非CHD组(n=52);将CHD组患者根据病情程度分为稳定型心绞痛组(n=82)、不稳定型心绞痛组(n=63)及急性心肌梗死组(n=41),根据冠状动脉造影结果将CHD组患者分为单支病变组(n=88)、双支病变组(n=60)及3支或以上病变组(n=38)。酶联免疫吸附试验测定各组患者血浆LPA和LP-PLA2水平,分析血浆LPA和LP-PLA2水平与冠状动脉病变程度的相关性。结果 急性心肌梗死组、不稳定型心绞痛组和稳定型心绞痛组患者血浆LPA、LP-PLA2水平及Gensini评分显著高于非CHD组(P<0.05),急性心肌梗死组和不稳定型心绞痛组患者血浆LPA、LP-PLA2水平及Gensini评分显著高于稳定型心绞痛组(P<0.05),急性心肌梗死组患者血浆LPA、LP-PLA2水平及Gensini评分显著高于不稳定型心绞痛组(P<0.05)。血浆LPA、LP-PLA2水平与Gensini评分呈显著正相关(r=0.577、0.609,P<0.05)。单支病变组、双支病变组和3支或以上病变组患者血浆LPA、LP-PLA2水平显著高于非CHD组(P<0.05),双支病变组和3支或以上病变组患者血浆LPA、LP-PLA2水平显著高于单支病变组(P<0.05),3支或以上病变组患者血浆LPA、LP-PLA2水平显著高于双支病变组(P<0.05)。结论 血浆LPA和LP-PLA2水平能够有效评估CHD的严重程度,并能够用于评估冠状动脉病变的进展。
Abstract:
Objective To evaluate the clinical significance of plasma lysophosphatidic acid (LPA) and lipoprotein associated phospholipase acid A2 (LP-PLA2) levels in patients with coronary atherosclerotic heart disease(CHD).Methods A total of 238 patients who were suspected coronary atherosclerotic heart disease in the Central Hospital of Zhumadian City from July 2015 to July 2017 were selected as the subjects.According to the clinical symptoms and examination results,the patients were divided into CHD group (n=186) and non-CHD group (n=52).The patients in the CHD group were divided into stable angina group(n=82),unstable angina group (n=63) and acute myocardial infarction group (n=41) according to the disease degree,and according to the coronary angiography results they were divided into single lesion group (n=88),the double branch lesion group (n=60) and three or more lesions group (n=38).Plasma LPA and LP-PLA2 levels were measured by enzyme-linked immunosorbent assay,and the correlation between plasma LPA and LP-PLA2 and severity of coronary artery disease was analyzed.Results The levels of plasma LPA,LP-PLA2 and Gensini scores in the acute myocardial infarction group,unstable angina group and stable angina group were significantly higher than those in the non-CHD group(P<0.05).The levels of plasma LPA,LP-PLA2 and Gensini scores in acute angina pectoris group and unstable angina group were significantly higher than those in stable angina group (P<0.05).The levels of plasma LPA,LP-PLA2 and Gensini scores in the acute myocardial infarction group were significantly higher than those in the unstable angina group (P<0.05).Pearson correlation analysis showed that plasma LPA and LP-PLA2 levels were positively correlated with Gensini score (r=0.577,0.609,P<0.05).The levels of plasma LPA and LP-PLA2 in the single vessel disease group,double branch lesion group and three or more lesions group were significantly higher than those in the non-CHD group (P<0.05).The levels of plasma LPA and LP-PLA2 in double branch lesion group and three or more lesions group were significantly higher than those in single lesion group (P<0.05).The levels of plasma LPA and LP-PLA2 in three or more lesions group were significantly higher than those in double branch lesion group (P<0.05).Conclusion LPA and LP-PLA2 levels can effectively assess the severity of coronary heart disease and can be used to evaluate the progression of CHD.
