摘要:
目的 探讨重组人促红细胞生成素(rhEPO)对早产儿脑损伤的防治作用。方法 选择2015年1月至2016年12月驻马店市中心医院收治的早产儿72例为研究对象。根据早产儿家属意愿及治疗方法分为观察组和对照组,每组36例。对照组早产儿给予保暖、维持血糖和血压、营养支持等常规治疗措施,观察组早产儿在常规治疗基础上给予rhEPO 250 IU·kg-1,静脉注射,每周3次,4周为1个疗程,共治疗3个疗程。分别于纠正胎龄40周(治疗前)及治疗后采用酶联免疫吸附试验法检测2组早产儿血清S100β、白细胞介素-6(IL-6)和促红细胞生成素(EPO)水平,2组早产儿分别于治疗前及治疗后采用新生儿行为神经测定(NBNA)评分法进行神经行为测试,并观察治疗期间2组早产儿并发症发生情况。结果 治疗前2组早产儿血清S100β、IL-6及EPO水平比较差异无统计学意义(P>0.05);2组早产儿治疗后血清S100β及IL-6水平显著低于治疗前(P<0.05),EPO水平显著高于治疗前(P<0.05);治疗后,观察组早产儿血清S100β及IL-6水平显著低于对照组(P<0.05),EPO水平显著高于对照组(P<0.05)。治疗前2组早产儿NBNA评分比较差异无统计学意义(P>0.05),2组早产儿治疗后NBNA评分显著高于治疗前(P<0.05);治疗后,观察组早产儿NBNA评分显著高于对照组(P<0.05)。对照组和观察组早产儿并发症发生率分别为36.11%(13/36)和13.89%(5/36),观察组早产儿并发症发生率显著低于对照组(χ2=4.741,P<0.05)。结论 rhEPO预防性应用可以有效改善早产儿神经系统功能,减轻脑损伤。
Abstract:
Objective To investigate the preventive effect of recombinant human erythropoietin(rhEPO) on brain injury in premature infants.Methods A total of 72 premature infants in Zhumadian Central Hospital from January 2015 to December 2016 were selected as the subjects.The premature infants were divided into observation group and control group according to their parental wishes and treatment methods,36 cases in each group.The premature infants in the control group were treated with conventional treatment measures such as keeping warm,maintaining blood sugar and blood pressure,nutritional support and so on.The preterm infants in the observation group were treated with rhEPO 250 IU·kg-1 by intravenous injection for three courses of treatment,three times a week,and four weeks as a course of treatment.The levels of serum S100β,interleukin-6(IL-6) and erythropoietin(EPO) were detected by enzyme-linked immunosorbent assay at 40 weeks of corrected gestational age (before treatment) and after treatment.The neurobehavioral status of the preterm infants was assessed by neonatal behavioral neurological assessment(NBNA) score at before and after treatment.The complications of the preterm infants in the two groups were observed during treatment.Results There was no significant difference in serum S100β,IL-6 and EPO levels between the two groups before treatment(P>0.05).The levels of serum S100β and IL-6 of the preterm infants after treatment were significantly lower than those before treatment in the two groups(P<0.05),and the serum EPO level of the preterm infants after treatment was significantly higher than that before treatment in the two groups(P<0.05).After treatment,the levels of serum S100β and IL-6 in the observation group were significantly lower than those in the control group(P<0.05),and the level of serum EPO in the observation group was significantly higher than that in the control group(P<0.05).There was no significant difference in the NBNA score between the two groups before treatment(P>0.05).The NBNA score of the preterm infants after treatment was significantly higher than that before treatment in the two groups(P<0.05).The NBNA score of the premature infants in the observation group was significantly higher than that in the control group after treatment(P<0.05).The incidence of complications in the control group and the observation group was 36.11%(13/36) and 13.89%(5/36) respectively,the incidence of complications in the observation group was significantly lower than that in the control group(χ2=4.741,P<0.05).Conclusion RhEPO prophylactic application can effectively improve neurological function and reduce brain damage in premature infants.
