摘要:
目的 探讨风湿性心脏病(RHD)患者二尖瓣瓣膜中辅助性T细胞(Th)17及Th22细胞的表达及临床意义。方法 选择新乡医学院第一附属医院心血管外科2016年5月至2017年5月确诊的40例RHD患者为观察组,另选择30例非风湿性心脏瓣膜病变患者为对照组。获取2组患者的瓣膜组织,应用流式细胞仪检测瓣膜组织中Th17及Th22细胞在CD4+T细胞中的比例;采用实时荧光定量反转录聚合酶链反应(qRT-PCR)检测瓣膜组织中白细胞介素(IL)-17和IL-22 mRNA的相对表达量;采用酶联免疫吸附试验检测瓣膜细胞培养上清液中IL-17和IL-22的水平。结果 观察组患者二尖瓣瓣膜组织中Th17和Th22细胞占CD4+T细胞的比例分别为(3.95±0.81)%和(3.24±0.68)%,对照组患者二尖瓣瓣膜组织中Th17和Th22细胞占CD4+T细胞的比例分别为(1.34±0.25)%和(0.83±0.20)%,观察组患者二尖瓣瓣膜组织中Th17和Th22细胞占CD4+T细胞的比例显著高于对照组 (P<0.05)。观察组患者瓣膜组织中IL-17及IL-22 mRNA相对表达量分别为5.03±1.43和4.67±1.12,对照组患者瓣膜组织中IL-17及IL-22 mRNA相对表达量分别为2.38±1.03和1.98±1.34,观察组患者二尖瓣瓣膜组织中IL-17、IL-22 mRNA相对表达量显著高于对照组(P<0.05)。观察组患者细胞培养上清液中IL-17和IL-22水平分别为(25.39±3.86)、(20.10±4.15)g·L-1,对照组患者细胞培养上清液中IL-17和IL-22水平分别为(8.34±2.13)、(10.13±2.15)g·L-1;观察组患者细胞培养上清液中IL-17、IL-22水平显著高于对照组(P<0.05)。结论 RHD患者二尖瓣瓣膜中Th17及Th22细胞比例及相应细胞因子的表达均明显升高,其在该疾病的发生、发展中可能起一定的促进作用。
Abstract:
Objective To investigate the expression and clinical significance of the helper T cell 17(Th17) and helper T cell 22(Th22) in the mitral valve of patients with rheumatic heart disease(RHD).Methods Forty patients with RHD were selected as the observation group;and thirty patients with nonrheumatic heart disease were selected as the control group.The valve tissues of patients in the two groups were obtained,and the proportion of Th17 cells and Th22 cells in the CD4+T cells in the mitral valve of the two groups were detected by flow cytometry;the expression of interleukin(IL)-17 and IL-22 mRNA in the valve tissues was detected by quantitative real-time polymerase chain reaction (qRT-PCR) and the levels of IL-17 and IL-22 in the cultured supernatant of valvular cells were detected by enzyme-linked immunosorbent assay.Results The proportion of Th17 cells and Th22 cells in the CD4+T cells in the mitral valve of patients in observation group was (3.95±0.81)% and(3.24±0.68)%,and in control group was (1.34±0.25)% and(0.83±0.20)%;the proportion of Th17 cells and Th22 cells in the CD4+T cells in the mitral valve of patients in observation group was significantly higher than that in the control group(P<0.05).The relative expression of IL-17 and IL-22 mRNA in the valve tissues of patients in the observation group was (5.03±1.43) and (4.67±1.12),and in the control group was (2.38±1.03)and(1.98±1.34);the relative expression of IL-17 and IL-22 mRNA in the valve tissues of patients in the observation group was significantly higher than that in the control group (P<0.05).The levels of IL-17 and IL-22 in the cultured supernatant of valvular cells in observation group were (25.39±3.86),(20.10±4.15) g·L-1 and in the control group were (8.34±2.13),(10.13±2.15)g·L-1;the levels of IL-17 and IL-22 in the cultured supernatant of valvular cells in observation group were significantly higher than those in the control group(P<0.05).Conclusion The proportion of Th17 and Th22 cells and the expression of related cytokines in the mitral valve of RHD patients are obviously increased,which may play a certain role in the occurrence and development of the disease.
