摘要:
目的 探讨磷酸肌酸钠联合喜炎平注射液治疗轮状病毒性肠炎(RE)合并心肌损害的临床效果及对患儿心肌酶谱水平的影响。方法 选择2013年11月至2016年12月新乡市妇幼保健院收治的RE合并心肌损害患儿106例,根据建档顺序分为对照组和观察组各53例。对照组患儿给予微生态制剂调节肠道菌群、纠正水电解质紊乱与酸碱失衡、调整饮食等常规对症治疗与干预,并口服蒙脱石散,同时给予磷酸肌酸钠粉针50 mg·kg-1静脉滴注,每日1次,疗程7 d。观察组患儿在对照组治疗基础上给予喜炎平注射液8 mg·kg-1静脉滴注,每日1次,疗程7 d。比较2组患儿的平均退热时间、止泻时间、临床疗效、并发症发生率及治疗前后心肌酶谱[乳酸脱氢酶(LDH)、天门冬氨酸氨基转移酶(AST)、磷酸肌酸激酶(CK)、磷酸肌酸激酶同工酶(CK-MB)]活性和炎性因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)]水平。结果 观察组患儿的平均退热时间、止泻时间均短于对照组(P<0.05)。观察组患者的治疗总有效率显著高于对照组(χ2=3.9750,P<0.05)。治疗前,2组患儿的血清LDH、AST、CK、CK-MB活性和IL-6、TNF-α、hs-CRP水平比较差异无统计学意义(P>0.05)。治疗后,2组患儿的血清LDH、AST、CK、CK-MB活性和IL-6、TNF-α、hs-CRP水平与治疗前比较均显著降低(P<0.05);观察组患儿的血清LDH、AST、CK、CK-MB活性和IL-6、TNF-α、hs-CRP水平均显著低于对照组(P<0.05)。观察组患儿的不良反应发生率与对照组比较差异无统计学意义(χ2=0.000 3,P>0.05)。结论 磷酸肌酸钠联合喜炎平注射液治疗可有效缓解RE合并心肌损害患儿的临床症状,减轻机体炎症反应,降低血清心肌酶谱水平,促进受损心肌细胞修复,且具有一定安全性。
Abstract:
Objective To study the clinical effect of creatine phosphate sodium combined with Xiyanping injection rotavirus enteritis(RE) in children complicated with myocardial injury and it′s influence on myocardial enzymes levels.Methods A total of 106 children with RE complicated with myocardial injury were selected from November 2013 to December 2016 in Xinxiang Maternal and Child Health Hospital.The children were divided into observation group and control group according to treatment method,53 cases in each group.The children in the control group were treated with conventional therapy:probiotics regulating intestinal flora and diet,correcting water electrolyte disturbance and acid-base imbalance,montmorillonite powder,and creatine phosphate sodium powder (50 mg·kg-1,intravenous drip,once a day for 7 days).Based on these,the children in the observation group were treated with Xiyanping injection 8 mg·kg-1 by intravenous drip,once a day for 7 days.The defervescence time,diarrhea cessation time,clinical effect,complications incidence,the levels of myocardial enzymes [lactate dehydrogenase (LDH) and aspartate aminotransferase (AST),creatine kinase (CK),creatine kinase isoenzyme (CK-MB)] and inflammatory factor[interleukin-6 (IL-6),tumor necrosis factor-α(TNF-α),high sensitive C reactive protein (hs-CRP)] before and after treatment were compared.Results The defervescence time and diarrhea cessation time in the observation group were significantly shorter than those in the control group(P<0.05).The total effective rate in the observation group was significantly higher than that in the control group(χ2=3.975,P<0.05).There was no significant difference in the levels of serum LDH,AST,CK,CK-MB,IL-6,TNF-α and hs-CRP between the two groups before treatment (P>0.05).The levels of serum LDH,AST,CK,CK-MB,IL-6,TNF-α and hs-CRP after treatment were significantly lower than those before treatment in the two groups (P<0.05).The levels of serum LDH,AST,CK,CK-MB,IL-6,TNF-α and hs-CRP in the observation group were significantly lower than those in the control group after treatment (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (χ2=0.000 3,P>0.05).Conclusion Creatine phosphate sodium combined with Xiyanping injection can effectively alleviate the clinical symptoms,relieve inflammatory reaction,decrease the levels of serum myocardial enzymes,and promote myocardial cell repair with high safety in patients with RE complicated with myocardial injury.
