摘要:
目的 探讨声速匹配组织量化(SVQ)技术在肝纤维化诊断中的应用价值。方法 分别对2014年6月至2015年12月在郑州大学第二附属医院就诊的慢性乙型病毒性肝炎患者79例(病例组)和同期体检健康者110人(对照组)应用SVQ技术测量感兴趣组织的区域速度指数,计算出相应的感兴趣区域内的声速(SV),同时以病理诊断结果作为金标准,构建受试者工作特征曲线,分析应用SVQ技术诊断肝纤维化的敏感度、特异度和准确度。结果 以对照组SV等于1 575 m·s-1做为界点时判断肝纤维化的敏感度、特异度、准确度、阳性预测值、阴性预测值分别为80.85%、84.38%、82.28%、88.37%、75.00%。将SV≥1 575 m·s-1作为诊断患有肝纤维化的标准时,曲线下面积为0.894[95%可信区间(0.826,0.963)],Youden指数为0.653。结论 SVQ技术对肝纤维化的诊断价值较高,SV越大,患肝纤维化的可能性越大。
Abstract:
Objective To explore the value of sound velocity tissue quantification (SVQ) technique in diagnosing of hepatic fibrosis.Methods Seventy-nine patients with chronic viral hepatitis B in the Second Affiliated Hospital of Zhengzhou University from June 2014 to December 2015 were selected as disease group.At the same time,one hundred and ten healthy volunteers were selected as control group.The zone speed index(ZSI) of the region of interest(ROI) was measured by SVQ technique and the sound velocity (SV) of the ROI was calculated.Then the receiver operating characteristic curve was constructed by using pathological results as gold standard.The sensitivity,specificity and accuracy of SVQ technique in diagnosis of hepatic fibrosis was analyzed.Results Take SV equal to 1 575 m·s-1 as the boundary point,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of SVQ technique in diagnosis of hepatic fibrosis was 80.85%,84.38%,82.28%,88.37%,75.00% respectively.When the SV≥1 575 m·s-1 was used as the standard for the diagnosis of hepatic fibrosis,the area under the curve was 0.894[95% confidence interval (0.826,0.963)],and the Youden index was 0.653.Conclusion SVQ technique has high value in the diagnosis of liver fibrosis.The greater the SV is the greater the possibility of patients with liver fibrosis.
