摘要:
目的 评价不同桩系统和剩余牙根组织量对根管治疗后牙根抗折强度的影响。方法 选择2014年5月至2015年5月于郑州市口腔医院因牙周病拔除的70颗新鲜上颌尖牙分为对照组、实验组1、实验组2、实验组3、实验组4、实验组5和实验组6,每组10颗。对照组牙根壁厚度为全厚,实验组1、实验组2、实验组3牙根壁厚度为 2 mm,实验组4、实验组5、实验组6牙根壁厚度为1 mm。对照组不做任何处理,实验组1、实验组4采用纤维桩(FP)进行修复;实验组2、实验组5 采用纤维桩外衬复合树脂(FPC)进行修复;实验组3、实验组6 采用镍铬合金铸造桩核(CPC)进行修复。所有牙齿均用金属冠修复并置于咀嚼模拟器上循环250 000次,然后用万能试验机测试所有样本的抗折裂强度。结果 实验组2、实验组5和对照组牙齿的平均折裂强度两两比较差异均无统计学意义(P>0.05);实验组1、实验组3、实验组4和实验组6牙齿的平均折裂强度低于对照组、实验组2和实验组5(P<0.05);实验组1、实验组3、实验组4和实验组6牙齿的断裂强度两两比较差异均无统计学意义(P>0.05)。各组折裂模式显示,CPC 修复的牙齿主要表现为不可修复性折裂,而FP 和FPC 修复的牙齿主要表现为可修复性折裂。结论 采用不同的桩系统对牙根的抗折强度影响很大,而剩余牙根壁的厚度对牙根的抗折强度影响很小。
Abstract:
Objective To investigate the influence of post system and the amount of remaining teeth root tissue on the fracture resistance of teeth after root canal therapy.Methods Seventy upper canine teeth which were pulled out because of periodontal disease in Zhengzhou Stomatologic Hospital from May 2014 to May 2015 were selected and divided into control group,experimental group 1,experimental group 2,experimental group 3,experimental group 4,experimental group 5,experimental group 6,with ten teeth in each group.The thickness of teeth root wall in the control group was complete;the thickness of teeth root wall in experimental group 1,experimental group 2 and experimental group 3 was 2 mm;the thickness of teeth root wall in experimental group 4,experimental group 5 and experimental group 6 was 1 mm.The teeth in the control group were not given any treatment;the teeth in experimental group 1 and experimental group 4 were repaired with fiber post(FP);the teeth in experimental group 2 and experimental group 5 were repaired with fiber post relined with resin composite(FPC);the teeth in experimental group 3 and experimental group 6 were repaired with cast Ni-Cr alloy post and core(CPC).All teeth were restored with metal crowns and placed on a chewing simulator for 250 000 cycles.The fracture resistance of all teeth were tested in a universal testing machine.Results There was no statistic difference in the average fracture resistance of teeth among the control group,experimental group 2 and experimental group 5 (P>0.05).The average fracture resistance of teeth in experimental group 1,experimental group 3,experimental group 4,experimental group 6 was significantly lower than that in the control group,experimental group 2 and experimental group 5(P<0.05).There was no statistic difference in the average fracture resistance of teeth among the experimental group 1,experimental group 3,experimental group 4 and experimental group 6 (P>0.05).The fracture mode of teeth in each group showed that the teeth repaired with CPC were mainly non repairable failure,while the teeth repaired with FP and FPC were mainly repairable failure.Conclusion The post system has a significant influence on fracture resistance.However,the remaining thickness of root canal of teeth has little effect on fracture resistance.
