摘要:
目的 通过检测多发性骨髓瘤(MM)患者骨髓中调节性T细胞(Treg细胞)和辅助性T细胞17(Th17细胞)的比例,分析Treg/Th17比值变化在MM发病机制中的作用及其与MM临床指标的关系。方法 采用流式细胞术对新乡医学院第一附属医院血液科46例MM患者(观察组)和20例健康体检者(对照组)骨髓单个核细胞进行检测,分析Treg细胞和Th17细胞的比例及Treg/Th17比值的变化。采用酶联免疫吸附试验检测骨髓中白细胞介素-10(IL-10)、转化生长因子-β(TGF-β)、白细胞介素-17(IL-17)和白细胞介素-6(IL-6)表达水平。同时,对患者血清β2-微球蛋白(β2-MG)、乳酸脱氢酶(LDH)、白蛋白(ALB)及血红蛋白(HGB)水平与Treg/Th17比值的相关性进行分析。结果 观察组患者骨髓中Treg细胞比例为(3.89±1.62)%,显著低于对照组的(12.07±3.03) %(P<0.05);Th17细胞比例为(1.72±0.67)%,显著高于对照组的(0.61±0.11)%(P<0.05)。观察组患者骨髓中Treg/Th17比值为4.87±1.71,显著低于对照组的17.07±4.17(P<0.05)。与对照组比较,观察组患者骨髓中IL-10和TGF-β水平显著下降,IL-17和IL-6水平显著升高(P<0.05)。MM 患者骨髓中Treg/Th17比值与β2-MG和LDH水平呈负相关(r=-0.681、-0.588,P<0.01),与ALB和HGB水平无明显相关性(r=0.126、0.067,P>0.05)。结论 MM患者骨髓中Treg细胞比例及相关细胞因子(IL-10和TGF-β)水平下降,Th17细胞比例及相关细胞因子(IL-17和IL-6)水平升高,Treg/Th17比值下降,并且与MM患者的部分临床指标(β2-MG和LDH)存在一定的相关性。
Abstract:
Objective To analyze the Treg/Th17 ratio change in the pathogenesis of multiple myeloma (MM) and its relationship with clinical indicators through the detection of the proportion of regulatory T(Treg) cells and T helper 17(Th17) cells in bone marrow of patients with MM.Methods The bone marrow mononuclear cells of 46 patients with MM(observation group) and 20 healthy individuals(control group) were detected by flow cytometry.The proportion of Treg cell and Th17 cell and Treg/Th17 ratio were analyzed.The expression levels of interleukin-10 (IL-10),transfer growth factor-β (TGF-β),interleukin-17 (IL-17) and interleukin-6 (IL-6) in bone marrow serum were detected by enzyme linked immunosorbent assay method.At the same time,the correlation between serum levels of β2-microglobulin (β2-MG),lactate dehydrogenase (LDH),albumin (ALB) and hemoglobin (HGB) and Treg/Th17 ratio was analyzed.Results The percentage of Treg cells in bone marrow of patients with MM in the observation group was (3.89±1.62)%,significantly lower than (12.07±3.03)% in the control group (P<0.05).In contrast,the percentage of Th17 cells in the observation group was (1.72±0.67)%,significantly higher than(0.61±0.11)% in the control group (P<0.05).The Treg/Th17 ratio in the observation group and control group was 4.84±1.71 and 17.07±4.17 respectively,the Treg/Th17 ratio in the observation group was significantly lower than that in the control group (P<0.05).Compared with the control group,the serum levels of IL-10 and TGF-β decreased significantly in the observation group (P<0.05),while IL-17 and IL-6 increased significantly (P<0.05).The bone marrow Treg/Th17 ratio of patients with MM was negatively correlated with the levels of LDH and β2-MG (r=-0.681,-0.588;P<0.01),with no significant correlation with the levels of HGB and ALB (r=0.126,0.067;P>0.05).Conclusion The Treg cells proportion and related cytokines (IL-10 and TGF-β) in the bone marrow of patients with MM is declined,while the Th17 cells proportion and related cytokines (IL-17 and IL-6) is elevated.In addition,the Treg/Th17 ratio in the bone marrow of patients with MM is decreased.There is a certain correlation between the Treg/Th17 ratio change and clinical indexes(β2-MG and LDH),suggesting that immune dysfunction plays an important role in the occurrence,development and prognosis of multiple myeloma.
