摘要:
目的　探讨阿托伐他汀对血脂正常患者阵发性心房颤动（PAF）的影响及机制。方法　选择2012年9月至2015年9月郑州大学第二附属医院收治的血脂正常的PAF患者120例，根据治疗方法将患者分对照组和观察组，每组60例。对照组患者给予阿司匹林和琥珀酸美托洛尔等常规治疗，观察组患者在常规治疗基础上给予阿托伐他汀治疗，2组患者均连续治疗6个月。分别于治疗前及治疗6个月后检测2组患者血清肿瘤坏死因子-α（TNF-α）、白细胞介素-18（IL-18）、超敏C反应蛋白（hs-CRP）、总胆固醇（TC）、低密度脂蛋白胆固醇（LDL-C）、三酰甘油（TG）及脂蛋白a（Lpa）水平；所有患者随访12个月，观察2组患者PAF发作、心力衰竭及猝死发生率。结果　随访12个月，观察组患者PAF、心力衰竭及猝死发生率显著低于对照组（P<0.05）；2组患者肝功能损伤发生率比较差异无统计学意义（P>0.05）。治疗前2组患者TC、LDL-C、TG及Lpa水平比较差异均无统计学意义（P>0.05）；对照组患者治疗前与治疗后TC、LDL-C、TG及Lpa水平比较差异均无统计学意义（P>0.05），观察组患者治疗后TC、LDL-C、TG及Lpa水平显著低于治疗前（P<0.05）；治疗后观察组患者TC、LDL-C、TG及Lpa水平显著低于对照组（P<0.05）。治疗前2组患者血清TNF-α、hs-CRP、IL-18水平比较差异均无统计学意义（P>0.05），2组患者治疗后血清TNF-α、hs-CRP、IL-18水平均显著低于治疗前（P<0.05），且观察组患者血清TNF-α、hs-CRP、IL-18水平显著低于对照组（P<0.05）。结论　阿托伐他汀可能通过降低血脂及炎性因子水平抑制血脂正常患者PAF发作。
Abstract:
Objective　To investigate the effect and mechanism of atorvastatin on paroxysmal atrial fibrillation（PAF）in patients with normal blood lipids.Methods　A total of 120 PAF patients with normal blood lipids were selected in the Second Affiliated Hospital of Zhengzhou University from September 2012 to September 2015.The patients were divided into control group and observation group according to therapeutic method，60 cases in each group.The patients in the control group were treated with aspirin and metoprolol succinate；based on the conventional therapy，the patients in the observation group were treated with atorvastatin；the patients were treated continuously for 6 months in the two groups.The levels of serum tumor necrosis factor-α（TNF-α），interleukin-18（IL-18），high sensitive C-reactive protein（hs-CRP），total cholesterol（TC），low density lipoprotein-cholesterol（LDL-C），triacylglycerol（TG）and lipoprotein-a（Lpa）were detected before and after treatment for 6 months in the two groups.All patients were followed up for 12 months，the incidences of PAF attack，heart failure and sudden death were observed in the two groups.Results　During the follow-up period of 12 months，the incidence of PAF，heart failure and sudden death in the observation group were significantly lower than those in the control group（P<0.05）.There was no significant difference in the incidence of liver injury between the two groups（P>0.05）.There was no significant difference in the levels of TC，LDL-C，TG and Lpa between the two groups before treatment（P>0.05）.There was no significant difference in the levels of TC，LDL-C，TG and Lpa before and after treatment in the control groups（P>0.05）.The levels of TC，LDL-C，TG and Lpa after treatment were significantly lower than those before treatment in the observation group（P<0.05）.The levels of TC，LDL-C，TG and Lpa in the observation group were significantly lower than those in the control group after treatment（P<0.05）.There was no significant difference in the levels of serum TNF-α，hs-CRP and IL-18 between the two groups before treatment（P>0.05）.The levels of serum TNF-α，hs-CRP and IL-18 after treatment were significantly lower than those before treatment in the two groups（P<0.05）.The levels of serum TNF-α，hs-CRP and IL-18 in the observation group were significantly lower than those in the control group after treatment（P<0.05）.Conclusion　Atorvastatin may inhibit PAF attack by lowering the levels of lipid and inflammatory factors in patients with normal blood lipids.
