摘要:
目的　探讨腹腔镜卵巢囊肿剥除术治疗卵巢囊肿的临床效果。方法　选择2015年7月至2016年2月成都市妇女儿童中心医院收治的卵巢囊肿患者121例，根据治疗方法分为腹腔镜手术组（68例）和开腹手术组（53例），腹腔镜手术组患者给予腹腔镜卵巢囊肿剥除术，开腹手术组患者给予传统开腹卵巢囊肿剥除术，比较2组患者的手术时间、术中出血量、术后首次下床活动时间、术后首次排气时间及住院时间，并对2组患者手术前后血清皮质醇、雌二醇（E2）、促黄体激素（LH）、促卵泡激素（FSH）、白细胞介素-6（IL-6）及C反应蛋白（CRP）水平进行比较。结果　2组患者手术时间比较差异无统计学意义（P>0.05）；腹腔镜手术组患者术中出血量显著少于开腹手术组（P<0.05），术后首次下床时间、术后首次排气时间及住院时间显著短于开腹手术组（P<0.05）。2组患者术前血清CRP、IL-6及皮质醇水平比较差异均无统计学意义（P>0.05），2组患者术后24 h血清CRP、IL-6及皮质醇水平均显著高于术前（P<0.05）；术后24 h，腹腔镜手术组患者血清CRP、IL-6及皮质醇水平均显著低于开腹手术组（P<0.05）。2组患者术前血清E2、LH及FSH水平比较差异均无统计学意义（P>0.05）；2组患者术后3个月血清E2水平显著低于术前（P<0.05），LH和FSH水平显著高于术前（P<0.05）；术后3个月，腹腔镜手术组患者血清E2水平显著高于开腹手术组（P<0.05），LH和FSH水平显著低于开腹手术组（P<0.05）。结论　腹腔镜卵巢囊肿剥除术创伤小，患者术后恢复快，对患者激素水平影响小。
Abstract:
Objective　To explore the clinical effect of laparoscopic ovarian cystectomy in the treatment of ovarian cysts.Methods　A total of 121 patients with ovarian cysts were selected in Chengdu Women and Children′s Central Hospital from July 2015 to February 2016.The patients were divided into laparoscopic operation group（n=68） and laparotomy group（n=53） according to treatment method.The patients in the laparoscopic operation group were treated with laparoscopic ovarian cystectomy，and the patients in the laparotomy group were treated with traditional laparotomy cystectomy.The operation time，intraoperative blood loss，postoperative first ambulation time，postoperative first anal exhaust time and hospitalization time were compared between the two groups.The levels of serum cortisol，estradiol （E2），luteinizing hormone （LH），follicle stimulating hormone （FSH），interleukin-6（IL-6） and C-reactive protein（CRP） were compared between the two groups before and after operation.Results　There was no significant difference in the operation time between the two groups （P>0.05）.The intraoperative blood loss in laparoscopic operation group was significantly lower than that in laparotomy group （P<0.05），the postoperative first ambulation time，postoperative first anal exhaust time and hospitalization time in laparoscopic operation group were significantly shorter than those in laparotomy group （P<0.05）.There was no significant difference in the levels of serum CRP，IL-6 and cortisol between the two groups before operation（P>0.05）.The levels of serum CRP，IL-6 and cortisol at the time point of 24 h after operation were significantly higher than those before operation in the two groups（P<0.05）.The levels of serum CRP，IL-6 and cortisol in laparoscopic operation group were significantly lower than those in laparotomy group at the time point of 24 h after operation （P<0.05）.There was no significant difference in levels of serum E2，LH and FSH between the two groups before operation （P>0.05）.The serum E2 level at the time point of three months after operation was significantly lower than that before operation （P<0.05），and the levels of serum LH and FSH at the time point of three months after operation were significantly higher than those before operation in the two groups （P<0.05）.Three months after operation，the serum E2 level in laparoscopic operation group was significantly higher than that in laparotomy group（P<0.05），and the levels of serum LH and FSH in laparoscopic operation group were significantly lower than those in laparotomy group （P<0.05）.Conclusion　Laparoscopic ovarian cystectomy in the treatment of ovarian cysts has the advantages of less trauma，faster postoperative recovery and less influence on the hormone level of patients.
