摘要:
目的 探讨声辐射脉冲成像技术(ARFI)在原发性干燥综合征(PSS)涎腺病变诊断中的价值。方法 收集2014年6月至2016年7月河南中医药大学第一附属医院收治的32例PSS患者为PSS组,另收集健康志愿者35例为对照组,采用声触诊组织定量分析法(VTQ)检测2组受检者腮腺及颌下腺横向剪切波速度(SWV),并绘制SWV诊断PSS患者涎腺病变的受试者工作特征(ROC)曲线。结果 PSS组患者腮腺、颌下腺组织横向 SWV均高于对照组(P<0.01)。ROC曲线显示,当SWV为2.21 m·s-1时,诊断PSS腮腺病变的敏感度为92.5%,特异度为86.4%;当SWV为2.19 m·s-1时,诊断PSS颌下腺病变的敏感度为83.7%,特异度为90.5%。结论 VTQ可以反映PSS患者的涎腺组织硬度及弹性异常,对PSS患者的涎腺病变具有较高的诊断价值。
Abstract:
Objective To explore the value of acoustic radiation force imaging(ARFI) in the diagnosis of sialadenosis in patients with primary Sjogren syndrome (PSS).Methods Thirty-two patients with PSS in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from June 2014 to July 2016 were selected as PSS group,another 35 healthy volunteers were selected as control group.The transverse shear wave velocity (SWV) of parotid gland and submaxillary gland were detected by virtual touch quantization (VTQ) technology,and the receiver operating characteristic(ROC) curve of SWV in the diagnosis of sialadenosis in patients with PSS was drawn.Results The transverse SWVs of parotid gland and submandibular gland in PSS group were significantly higher than those in control group(P<0.01).The ROC curve showed that when the SWV was 2.21 m·s-1,the sensitivity to the diagnosis of parotid lesions in PSS patients was 92.5%,and the specificity was 86.4%;when the SWV was 2.19 m·s-1,the sensitivity to the diagnosis of submandibular gland lesions in PSS patients was 83.7%,and the specificity was 90.5%.Conclusion VTQ technique can reflect the hardness and elasticity abnormality of salivary gland tissue in patients with PSS,which has the high diagnostic value for sialadenosis in patients with PSS.
